Primary Responsibilities:

• Deliver in-home physical therapy assessments and develop individualized treatment plans
  
• Guide patients toward mobility and independence through hands-on therapy
  
• Coordinate care with interdisciplinary team members and family caregivers
  
• Maintain timely, compliant documentation and progress tracking
  

Requirements:

• Valid Physical Therapy license in Minnesota
  
• Strong interpersonal and clinical assessment skills
  
• Valid driver's license, reliable transportation, and current auto insurance
  
• Experience in home health preferred but not required
