Your Role as a Home Health Physical Therapist

As a Remedy PT, you will:

• 
  

  Provide personalized in-home therapy sessions tailored to each patient’s care plan.

  
  


• 
  

  Design treatment programs that enhance strength, balance, mobility, and fall prevention.

  
  


• 
  

  Educate patients and caregivers on safe movement techniques and daily living activities.

  
  


• 
  

  Document patient care and progress accurately using WellSky/Kinnser EMR.

  
  


• 
  

  Collaborate with physicians, case managers, and the interdisciplinary care team to ensure optimal outcomes.

  
  

What We’re Looking For

• 
  

  Licensed Physical Therapist (PT) in the state of California (required)

  
  


• 
  

  AHA CPR Certification (current)

  
  


• 
  

  1+ year of experience (home health experience preferred, but we will train the right candidate!)

  
  


• 
  

  Reliable transportation, valid driver’s license, and current auto insurance

  
  


• 
  

  Excellent communication and documentation skills

  
  


• 
  

  Compassionate, independent, and self-motivated professional
