Home Health Physical Therapist (PT)

Windsor Home Health is looking for a home health Physical Therapist on a Part-time basis and/or PRN. Our employees are passionate about improving our patients' quality of life, and delivering compassionate care. We believe in teamwork and value each and every employee.

  Your typical day may include, but is not limited to:

  
  
  
  
  • Interdisciplinary discussion/review of patients on schedule and treatment.
  
  
  • New admissions evaluations.
  
  
  • Review of potential discharges.
  
  
  • Staff training and/or communication of patient progress.
  
  
  • Programming: Wheelchair, splints, adaptive equipment.
  
  
  • Communication with family of patients with progress/updated goals/adaptive equipment/change of status.
