Senior Physical Therapist - Home Health (Up to $115K)

We're seeking a compassionate and skilled Senior Physical Therapist to join our home health team in Taylor, TX. In this role, you'll provide personalized therapeutic care to patients in their homes, helping them regain mobility, independence, and quality of life. This position offers competitive compensation ranging from $105,000 to $115,000 annually.

Core Responsibilities:

• Perform detailed physical assessments and evaluate patient conditions through comprehensive medical history reviews
• Develop and execute customized treatment plans aligned with patient-specific goals
• Deliver therapeutic interventions and exercise programs to optimize recovery outcomes
• Provide patient and family education on injury prevention and rehabilitation techniques
• Track and document patient progress using electronic health records (EHR)
• Coordinate care plans with physicians, nurses, and other healthcare professionals
• Ensure compliance with all healthcare regulations and documentation requirements

Required Qualifications:

• Master's degree in Physical Therapy from an accredited institution
• Current state Physical Therapy license in good standing
• Minimum 1 year of home health physical therapy experience
• Valid CPR certification
• Current driver's license and reliable transportation
• Strong interpersonal and communication skills
• Proficiency in EHR systems and documentation
