Essential Duties & Responsibilities:

• Deliver exceptional therapeutic care to patients in their homes following established treatment plans
• Perform therapeutic exercises and physical modalities to improve patient mobility and function
• Document patient progress and maintain detailed electronic health records
• Collaborate with Physical Therapists and interdisciplinary healthcare teams
• Educate patients and caregivers on proper exercise techniques and home safety
• Assess and report changes in patient condition to supervising therapist
• Maintain equipment and ensure compliance with safety protocols

Required Qualifications:

• Associate's Degree from an accredited PTA program
• Current state PTA license in good standing
• 1+ year of home health experience preferred
• Valid driver's license and reliable transportation
• Current CPR certification
• Strong communication and interpersonal skills
