Home Health Registered Nurse (RN)

We are seeking a compassionate and dependable Home Health RN to provide skilled nursing care to patients in their homes. The RN will assess patient needs, coordinate care plans, and deliver treatments that promote recovery, independence, and quality of life. 

Key Responsibilities:  

• 
  

  Perform patient assessments and develop individualized care plans. 

  
  

• 
  

  Administer medications, wound care, and treatments as prescribed. 

  
  

• 
  

  Educate patients and families on disease management, recovery, and ongoing care needs. 

  
  

• 
  

  Collaborate with physicians, therapists, and other healthcare providers to ensure continuity of care. 

  
  

• 
  

  Document patient progress and maintain accurate records. 

  
  

• 
  

  Monitor changes in patient conditions and adjust care plans accordingly. 

  
  

Qualifications:  

• 
  

  Associate’s or Bachelor’s Degree in Nursing 

  
  

• 
  

  Current Registered Nurse (RN) license in KS 

  
  

• 
  

  Basic Life Support (BLS) certification, COVID vaccine, TB test 

  
  

• 
  

  Minimum 1 year of home health or acute care nursing experience 

  
  

• 
  

  Valid driver’s license and reliable transportation
