Home Health Registered Nurse, Valencia County - $7,500 Sign on Bonus

About the Role:
At Corus Home Health in Albuquerque, we’re looking for a compassionate, self-driven Registered Nurse to provide skilled home health services to patients throughout Valencia County. Key Responsibilities:

• Deliver professional nursing care in patients' homes based on physician orders
   
• Complete comprehensive assessments and develop individualized care plans
   
• Monitor patient progress and adjust care plans as needed
   
• Educate patients and families on health conditions, treatment, and self-care
   
• Coordinate care with interdisciplinary team members and external providers
   
• Maintain accurate clinical documentation using EMR systems
   
• Support compliance with regulatory and safety standards
   
• Participate in case conferences and ongoing quality initiatives
  

What We’re Looking For:

• Current RN license in the state of New Mexico
   
• At least one year of experience in a home health or acute care setting
   
• Strong clinical judgment and ability to work independently
   
• Excellent communication and time management skills
   
• Reliable transportation and willingness to travel within Valencia County
   
• A heart for community-based care and a commitment to patient dignity
