Home Health Registered Nurse - York County

We are seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team on a full-time, per-visit basis. This position is responsible for delivering high-quality, patient-centered nursing care in the home setting, including admissions, discharges, recertifications, and routine skilled nursing visits.

The ideal candidate is an experienced RN with strong clinical judgment, excellent communication skills, and the ability to work independently in a fast-paced, autonomous environment. At least 2 years of nursing experience is preferred, along with a background in wound care, IV therapy, and/or med-surg nursing.

Responsibilities:

• 
  

  Conduct skilled nursing visits in patients’ homes according to physician orders and individualized care plans.

  
  


• 
  

  Complete home health admissions, recertifications, discharges, and routine visits, ensuring timely and accurate documentation.

  
  


• 
  

  Provide direct care such as wound care, medication administration, IV therapy, catheter care, and chronic disease management.

  
  


• 
  

  Educate patients and families on disease processes, medications, and self-care techniques to support recovery and independence.

  
  


• 
  

  Communicate and coordinate care with physicians, therapists, and other members of the interdisciplinary team.

  
  


• 
  

  Ensure compliance with agency policies, Medicare/Medicaid regulations, and all applicable healthcare standards.

  
  

Qualifications:

• 
  

  Current RN license in the state of South Carolina

  
  


• 
  

  Minimum 2 years of nursing experience preferred (home health experience a plus).

  
  


• 
  

  Preferred experience in wound care, IV therapy, and/or medical-surgical nursing.

  
  


• 
  

  Strong assessment, critical thinking, and organizational skills.

  
  


• 
  

  Reliable transportation and valid driver's license.

  
  


• 
  

  Ability to work independently with minimal supervision.

  
  

Compensation:

• 
  

  Per-visit pay structure based on visit type (admissions, recerts, discharges, routine visits).

  
  


•
