Your Role as a RN:

We are seeking a Registered Nurse (RN) Case Manager to provide skilled nursing care to our home health patients. This position is available as full-time or part-time, with flexible scheduling.

You’ll work closely with an interdisciplinary team to:

• Conduct admission and OASIS assessments


• Perform head-to-toe and wound evaluations


• Create and implement individualized plans of care


• Provide follow-up skilled nursing visits


• Educate patients and caregivers


• Coordinate care with MDs, CNAs, PT/OT, and family members


• Attend weekly team meetings and contribute to care planning

Pay and Benefits

• $60 to $110 per visit


• Mileage reimbursement

Qualifications:

• Current Arizona RN license
  


• Reliable transportation


• Excellent communication and interpersonal skills

Preferred Qualifications:

• You live in Phoenix or the East Valley (Chandler, Gilbert, Mesa, etc.)


• You have prior home health experience
