• Develop and manage personalized care plans to support optimal health outcomes
• Collaborate with physicians, therapists, and caregivers to ensure quality of care
• Provide education and emotional support to clients and families
• Keep accurate, timely documentation in compliance with state and agency standards

• Active Minnesota RN license
• At least 2 years of nursing experience, including 1 year in home care
• Valid driver’s license and auto insurance
