Are you a compassionate, experienced RN ready to step into a more independent and rewarding role? Advanced Home Health and Hospice of Northern Nevada is seeking a Full-Time RN Case Manager to provide skilled nursing care and coordinate services for patients in the comfort of their homes.

This is a field-based position ideal for a nurse who thrives on autonomy, flexibility, and meaningful patient relationships.

What You’ll Do:

• Conduct initial and ongoing assessments, including OASIS documentation
• Create and manage individualized care plans in collaboration with an interdisciplinary team
• Coordinate with physicians, therapists, aides, and families to ensure cohesive care
• Supervise and support field staff (RNs, LPNs, Therapists, HHAs)
• Maintain timely, accurate documentation and communicate effectively with care teams

What You Bring:

• Active RN license (Nevada)
• Minimum 2 years of nursing experience (home health preferred)
• Strong critical thinking, time management, and communication skills
• Reliable transportation and a valid driver’s license
• Familiarity with home health documentation and regulatory standards
