Home Health RN - Chicago -Cermak to Devon, Lake Shore Drive to Harlem

Need a RN who enjoys working with patients and families in their home to teach about medications, self-management of disease process and instructing patient and caregiver how to infuse IV's, care of central lines, chemotherapy disconnects, foley care, pleurix care, wound care.  If you enjoy coordinating care with physicians, other team members, and referral sources.  You will help patient prevent re-hospitlization, and promote individualized patient care goals.  Must be RN licensed in Illinois and willing to cover Chicago North side city such as Near North, Goose Island, West Town, Ukrainian Village, Garfield Park, Humboldt Park, Wicker Park, Logan's Square, Roscoe Village, Hermosa, Galewood, Lake view, Lincoln Park, Clybourn Corridor, Humboldt Park, Lakeview, Logan's Square, Edgewater, Uptown, and Bucktown.
