Are you an experienced Registered Nurse (RN) with a heart for providing compassionate care? We're actively seeking a dedicated RN to join our team for a 13-week contract in home health. If you have recent experience in home health, proficiency in OASIS and Homecare Homebase (HCHB), solid clinical skills, and a genuine commitment to patient well-being, we want to hear from you!

Your Responsibilities:

• Collaborate with interdisciplinary teams to develop personalized home care treatment plans.
• Document patient assessments and progress using OASIS and HCHB systems.
• Administer medication and provide wound care as needed.
• Ensure accurate documentation and reporting utilizing OASIS and HCHB expertise.

Requirements:

• Active RN licensure in Louisiana.
• Minimum one year of recent home health experience.
• Proficiency in OASIS and Homecare Homebase (HCHB) systems.
• Current BLS Certification from the American Heart Association.
• Previous experience managing a home-to-home workload is advantageous.
