Registered Nurse (RN) Homecare Admissions opening full time 40 hrs. a week. This position is Monday - Friday 8:00am - 4:30pm with weekend / holiday rotations.
About the Job
As a Registered Nurse in Homecare or Hospice, you’ll deliver skilled, compassionate nursing care directly in the patient’s home. You’ll complete comprehensive assessments—including OASIS—develop individualized care plans, and collaborate closely with physicians, families, and the interdisciplinary team to ensure highâquality, goalâdirected, costâeffective care. This role blends clinical expertise, autonomy, and relationshipâcentered care to support patients in maintaining comfort, safety, and independence at home.
What You’ll Do
Complete initial and ongoing home assessments, including full physical evaluations and medical history reviews.
Use the nursing process to plan, deliver, and evaluate skilled nursing care.
Perform accurate OASIS assessments and ensure documentation reflects patient status and care needs.
Develop individualized care plans with measurable goals, involving patients and families in all planning and revisions.
Coordinate interdisciplinary care, communicating significant changes to physicians and obtaining necessary orders.
Teach and counsel patients and families on selfâcare techniques, disease management, and safety.
Prepare timely, accurate clinical notes and maintain strong communication with the care team.
Support quality outcomes, patient satisfaction, and appropriate utilization of visits.
Participate in staff meetings, case conferences, and quality improvement activities.
Maintain professional growth through continuing education and annual mandatory training.
Precept students when requested and provide excellent customer service.
Travel within assigned territory, occasionally assisting outside the usual area.
Perform other duties as assigned.
Who You Are
A Registered Nurse with an Associate Degree in Nursing (BSN preferred).
Licensed and in good standing with the New Hampshire Board of Nursing.
Equipped with at least one year of recent acute care experience.
Certified in BLS/CPR and able to meet driving, insurance, and background requirements.
Skilled in patient assessment, care planning, and communication.
Comfortable working independently in the home setting while collaborating with an interdisciplinary team.
Familiar with OASIS, home health reimbursement, and EMR systems (Allscripts preferred).
