Job Description - Home Health RN LPN

Job Title: Registered Nurse (RN) or Licensed Practical Nurse (LPN) – Home Health

Location: Southeast Georgia (Savannah & Surrounding Counties)

Position Overview:
We are seeking a dedicated and skilled RN or LPN to provide nursing care to clients in their homes. The ideal candidate will have a passion for patient-centered care, strong clinical judgment, and the ability to work independently while following established care plans.

Key Responsibilities:

Perform initial and ongoing client assessments.

Develop and update individualized care plans.

Administer medications, treatments, and wound care as ordered.

Monitor and document changes in patient condition.

Educate clients and families on health management and care techniques.

Collaborate with physicians, therapists, and other members of the care team.

Ensure compliance with all federal, state, and company regulations.

Qualifications:

Current GA RN or LPN license in good standing.

Minimum 1 year of clinical experience (home health experience preferred).

Current CPR/First Aid certification.

Excellent communication and organizational skills.

Reliable transportation and valid driver’s license.

Ability to work independently and manage multiple clients.

Schedule & Pay:

Flexible scheduling (full-time, part-time, and PRN available).

Competitive pay based on experience.

Mileage reimbursement for travel between clients.
