In This Role, You Will:

• Complete in-home nursing assessments and interventions
  
• Carry out care plans involving wound care, med administration, and symptom management
  
• Provide caregiver support and education on patient conditions
  
• Work closely with providers, therapists, and coordinators
  
• Document visits and care activities in a compliant and timely manner
  

Required Qualifications:

• Licensed RN in Minnesota with current CPR certification
  
• 1 or more years of direct RN experience
  
• Valid driver's license, reliable transportation, and insurance
  
• EMR experience preferred
  
• Commitment to compassionate, high-quality patient care
