Job Title:

Home Health RN - Weekend

Location:

Carnegie, PA 15106

Shift:

Full-time (40 hrs./week), Weekends Required

Salary:

$42.00 per hour
Responsibilities:
Manage and deliver patient care plans in the home health setting.
Conduct patient assessments, develop care plans, and provide skilled nursing care to patients in their homes.
Administer medications, treatments, and wound care as prescribed.
Monitor patient health status and update care plans as needed.
Educate patients and their families on disease management and treatment options.
Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
Document all patient care activities accurately and timely.
Maintain confidentiality and comply with all regulatory requirements.
Requirements:

Education:

Associate degree in Nursing (Bachelor's preferred).
Licensure:

Active Registered Nurse (RN) license in the state of Pennsylvania.
Experience:

At least one year of experience as an RN, preferably in home health or a related field.
Skills:

Strong clinical assessment and nursing skills.
Compassionate and patient-centered care approach.
Effective communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Excellent organizational and time management skills.
Proficiency in electronic medical records (EMR) systems.

Availability:

On-call availability required during weekends.
Travel:

Local travel required to patient locations in Carnegie/Crafton, PA and surrounding areas.
