Responsible for generating referrals for home care by building relationships with physicians, hospitals, long term care, independent and assisted living facilities and other community resources.
Maintains key relationships and partnerships with physicians, case managers, managed care insurance networks, social workers, area sub-acute and acute hospital sources.
Maintains a thorough knowledge of facilities, services, and acuity trends.
Identifies new partnerships and develops strategy for establishing referral relationships with these sources.
Acts as a consultant to referral sources and potential referral sources in the determination of placement in the healthcare continuum. Offers alternatives to suggested care when applicable.
Develops, conducts, and participates in external marketing events to promote all of Caregiversâ services; participates in exhibits, tours, and healthcare events.
Educates clients, family members, and significant others, referral sources, hospital personnel, and external case managers regarding all of the services that Caregiversâ provides.
Communicates with members of referred patientâs social/support base as needed. Communicates with other members of the current interdisciplinary team, establishes preliminary care plan/discharge plan and communicates to admission/social work for consideration.
Assists in developing market plans and strategies on-site and via telephone and also generates activity reports.
Support business development activities and help establish strong relationships with new and existing referral sources.
Requirements
Education:
Graduation for High School or equivalent. College degree preferred.
Training:
At least 2 years in home health or closely related health entity
Market knowledge of area hospitals, home health providers, skilled nursing, sub-acute care, and long-term care providers.
Prior sales/marketing experience in the health care industry, preferably in home health care
Formal sales training
Proven ability to develop and implement a sales and marketing plan
Evidence of achieving referral goals within the market
Excellent oral/written communication and interpersonal skills
Professional Experience:
This position requires an individual who is highly motivated, results driven, and able to develop and build strong, lasting relationships. Excellent planning, organization and presentation skills are critical. Recent health care marketing or sales experience is required. The ideal candidate will have established health care contacts and be able to network readily in the community.
Computer Skills:
Kinnser or other online clinical reporting system preferred.
