Key Responsibilities:
Understand frequency and accurately schedule field staff based on the frequency obtained from the Plan of Care.
Create and maintain patient and staff schedules according to the Plan of Care.
Initiate authorizations for patient care.
Track and review expiring authorizations.
Manage field staff schedules and be able to effectively communicate with field staff, patients, physicians and families regarding schedules and care coordination.
Responding to phone calls and inquiries from patients, staff and providers.
Ensuring appropriate staffing levels based on patient needs and authorized visits.
Coordinating with staff to ensure proper care delivery and continuity of care.
Adhere to Medicare guidelines and regulations for home health care.
Work closely with the Intake/Admission Supervisor.
Work closely with the Director of Nursing to effectively manage LUPAs and Utilization Review.
Cross train in Intake/Admissions and Human Resources.
Job Requirements
High School Diploma or equivalent
Must have strong organizational skills
Must be able to multi-task
Must have strong time management skills
Must have strong customer service skills
Must be able to work efficiently and accurately in Kinnser/WellSky home health EMR
Ability to adapt and find solutions in a sometimes rapidly changing environment.
Be able to work in a busy, fast paced environment
Minimum of 1 year working as a home health scheduler in a Medicare Certified Agency
