Job Overview:
We are seeking a dedicated and experienced Registered Nurse (RN) to join our Home Health Agency. The ideal candidate will play a key role in initiating new cases, utilizing OASIS (Outcome and Assessment Information Set), and demonstrating a comprehensive understanding of Medicare Home Health regulations. The RN will be responsible for delivering high-quality, patient-centered care in the comfort of patients' homes, ensuring compliance with all relevant healthcare standards.

Responsibilities:

Case Initiation:
- Start and manage new cases by conducting thorough assessments of patients' health status and developing individualized care plans.
- Collaborate with healthcare professionals to ensure a seamless transition of care from hospital to home.

OASIS Utilization:
- Proficiently use OASIS to document patient assessments, outcomes, and interventions accurately.
- Ensure compliance with OASIS documentation standards and maintain up-to-date patient records.

Medicare Home Health Compliance:
- Stay current on Medicare Home Health regulations and guidelines.
- Implement and adhere to Medicare requirements for documentation, billing, and quality assurance.

Patient-Centered Care:
- Provide compassionate and evidence-based nursing care to patients in their homes.
- Educate patients and their families on healthcare procedures, medications, and overall wellness.

Collaboration and Communication:
- Work closely with interdisciplinary teams, including physicians, therapists, and support staff.
- Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with care plans.

Qualifications:

Active and unrestricted Registered Nurse (RN) license in the state of Virginia.

Minimum of 2 years of nursing experience, with a focus on home health.

Proficient in OASIS documentation.

Strong understanding of Medicare Home Health regulations and compliance.

Excellent communication and interpersonal skills.

Ability to work independently and as part of a collaborative team.
