Home Health Social Worker (MSW)

About the Role
At Corus Home Health in Albuquerque, we’re looking for an experienced Social Worker (MSW or LCSW) to provide compassionate, person-centered care to patients in their homes. This is a highly autonomous role where you'll have the flexibility to manage your day while making a real impact in people’s lives.

Key Responsibilities

• Deliver social work services to home health patients per physician orders and individualized care plans
   
• Help patients and families navigate emotional, social, and practical challenges
   
• Participate in interdisciplinary care planning and patient case conferences
   
• Coordinate care with community resources and advocate for patients' needs
   
• Educate patients, families, and team members on psychosocial issues related to illness
   
• Document visits, care plans, and progress notes accurately and on time
   
• Maintain confidentiality and adhere to Medicare and agency guidelines
   
• Contribute to quality improvement and ongoing education efforts
   

 What We’re Looking For

• Master’s degree in Social Work (MSW) from an accredited program  
• Active New Mexico license   
• Minimum 1 year of social work experience in a healthcare setting; home health experience preferred  
• Proven ability to work independently and exercise sound clinical judgment  
• Strong communication and relationship-building skills  
• Knowledge of Medicare regulations related to social work services  
• A valid driver’s license and reliable transportation
