Your Role

As our Home Hospice Social Worker, you will provide outpatient hospice services at patients' homes, assisted living, and skilled nursing facilities. Your empathetic communication skills and deep understanding of the hospice care philosophy will serve as a beacon of hope and comfort to our patients and their families.

Our Ideal Candidate

• Holds an MSW degree 


• Possesses a valid state driver's license, automobile liability insurance, and dependable transportation


• Has at least one year of social work experience in a healthcare setting, preferably hospice


• Is a compassionate communicator with excellent written and verbal skills


• Demonstrates an ability to build rapport with patients, families, and interdisciplinary team members


• Understands and accepts the hospice philosophy of care

Your Contribution

As a Home Hospice Social Worker, you'll be a beacon of hope, providing counseling services per physicians' orders, and assisting in navigating the network of community resources. Your responsibilities will include evaluating patients'/families' coping skills, aiding in discharge planning, and contributing to individual and group bereavement counseling. Furthermore, you will maintain clinical records in accordance with Inspire Hospice's policies and procedures.
