Job Title: Home Visiting Support Specialist


Reports To Program Manager


Hours: 37.5 hours/week


FLSA Status: Full-time, Non-exempt


Location: Lackawanna and Luzerne Counties

Summary:


Member of Outreach staff with primary responsibility for engaging in home visitation, providing evidence-based curricula (Parents as Teachers and SafeCare Augmented), community-based education, and related support services.  Provide home visits and education to up to 18 participating families.

Duties:



  • Makes home visits to pregnant moms and families with children aged birth


through age 5, providing support in the areas of: Parent Child Interaction, Family Well-being, Development Centered Parenting, Health, and Safety in both Lackawanna and Luzerne Counties.



  • Maintains related files/record keeping and completes program-related data entry.

  • Plans individual and group activities that promote parents as their children’s


first teacher to stimulate growth in children’s language, social, and motor skills, such as learning to listen to instructions, playing with others, and using play equipment. Parents include all caregivers: teens, fathers,  grandparents, foster parents, etc.



  • Collaborates with community partners and human service agencies (local


Children & Youth agency) to link families to resources. Provides participant referral to schools, early intervention programs, Scranton Counseling  Center, etc., as needed.



  • Provides follow-up to families as needed; maintains appropriate record keeping (state or agency related).

  • Participates in team process focused on best practices to serve participating families, the community, and Outreach.

  • Reliable transportation, a current driver’s license, and insurance are required

  • Completion of certification in Parents as Teachers and SafeCare Augmented training is mandatory.

  • Some evening and weekend hours are required.

  • Attends community outreach events as needed in both Lackawanna and Luzerne Counties.


 



  • Participates in in-person team meetings, in-person reflective supervision, and home visit observations to ensure program fidelity

  • In-person, bimonthly all-staff meetings are mandatory

  • Daily in-office attendance is required.

  • Performs other duties as assigned.



Qualifications:

Education/Experience: 


Bachelor's degree (BA/BS) in social work, human development, psychology, neuroscience, early childhood education, or a related field from a four-year college or university. Familiarity with working with diverse cultures/families with economic challenges is a must. Ability to teach relevant programming using the PAT  Foundational & Model Implementation Curriculum and the Safe Care Augmented Model Curriculum to participants as assigned.  Good understanding of family dynamics, child development, and psychology.  Mandatory certification training is required (expenses paid by Outreach). 



Computer Skills: 


To perform this job successfully, an individual should have general computer skills, literacy skills, and experience with data collection software and MS Office. 


 


Work Environment:


The noise level in the work environment is usually moderate. Home environments where children live can be unpredictable, however, and subject to constant change. 



Physical Demands:


While performing the duties of this job, the employee is occasionally required to stand, walk, sit(on the floor), stoop, kneel, crouch, or crawl as well as talk and hear clearly, as all may be necessary in instructing educational activities within a home environment. These components may be needed during home visits, parent/child group activities, toddler playgroups, etc. The employee must occasionally lift and/or move up to 25 pounds while transporting materials, etc.
