HOPE Project Support Staff

JOB SUMMARY: Coordinate with clinical and business departments the collaborative process of grant compliance. Screen and track potential clients for acceptance, assist with development of care team and resources for client success, and effectively communicate across departments.

ESSENTIAL FUNCTIONS: 

• Provide support to the Director of Clinical Services and Performance, Chief Operating Officer and Social Workers to ensure grant compliance.
• Screen for client enrollment based on inclusion and exclusion criteria.  
• Support Social Workers in case coordination to develop care team. 
• Plan and coordinate a package of incentives to support ongoing client engagement based on individualized client’s needs.
• Support Discharge Planner to ensure access to resources upon transition from residential to outpatient level of care for clients.
• Monitors, tracks and completes client surveys to comply with the Government Performance and Results Act (GPRA) standards throughout client’s year-long treatment at admission, 6 months and 1 year.
• Advocate with internal and external resources to enhance the accessibility, continuity, and efficiency of the HOPE project grant. 

• Minimum of 2 years of human services experience with preference for those with experience working in addiction field. 
• Must be at least 21 years of age. 
• Current Driver’s License and good driving record. 
• Must be CPR certified within 90 days of hire. 
• Self-identified person in recovery from a Substance Use Disorder must have a minimum of two years of sobriety from substances of abuse. 
• Must pass pre-hire requirements including a criminal background check, UA, and TB test.
