We are looking for an experienced, talented, and compassionate Hospice Admissions Coordinator for a full-time position located in our Norwell Office. 


 


What will you be doing: 


The Hospice Admissions Coordinator will support the Hospice Clinical Managers, triage and business development team in the coordination and scheduling of Hospice admissions and follow up visits as needed. Assist in the communication and coordination of quality services of the mufti-disciplinary team for safe and exceptional patient/client care.


 



  • Works with Business Development, Intake and Hospice managers to efficiently, effectively and timely coordinate Hospice admissions and follow up visits as needed.

  • Triage calls from families and facilities for appropriate course of action and follow up for the scheduling of admission visits or other needs as necessary.

  • Ensures the documentation required for reimbursement of care is completed on a timely basis and in compliance with state and federal regulations.

  • Performs duties as necessary to maintain quality clinical standards.

  • Represents the agency at various committees in the agency and the community as well as occasional industry-specific conferences, meetings, seminars, etc.

  • Assists in ordering DME and medications as needed.

  • Monitors patient clinical records concurrently and retrospectively utilizing established outcome criteria and to support customer satisfaction scores.

  • Supports CTA's to ensure compliance with visit frequency and visit necessity.

  • Assures agency compliance with all federal, state and local laws as well as adhering to the Norwell Visiting Nurse Association, Inc. Corporate Compliance policy in the prevention of fraud and abuse on behalf of the Agency.

  • Assign caseloads based on location/need and ability to work as a collaborative part of interdisciplinary team.


 


What we’re looking for in you:



  • 3+ years of clinical administrative or coordination of care experience

  • Home care experience strongly

  • Hospice experience strongly preferred.

  • Strong clinical assessment skills and the ability to prioritize and triage patients.

  • Exceptional communication skills and the ability to work within a team structure.

  • Excellent organizational skills and time management skills.

  • Ability to work independently and understanding of how to access resources for patients.

  • Strong computer skills

VACCINE REQUIREMENTS: NVNA and Hospice requires full vaccination for Covid-19 and flu as a condition of employment. An exception to this mandatory requirement can be requested.
