Advanced Home Health and Hospice of Kansas City is seeking a dedicated Hospice Admissions Nurse to guide patients and families through the start of their hospice journey with care, clarity, and compassion.

About the Role

As our Hospice Admissions Nurse, you will be the face of our agency—educating patients and families, completing assessments, and initiating personalized care plans that ensure comfort and dignity from day one.

Schedule: Monday through Friday, 8:30 AM to 4:30 PM
Employment Type: Full-Time
Location: Kansas City, MO and surrounding areas

Key Responsibilities

• Conduct comprehensive in-home hospice admissions and evaluations
• Provide clear education on hospice services, eligibility, and philosophy 
• Collaborate with physicians, referral sources, and interdisciplinary team members 
• Initiate and coordinate the development of individualized plans of care
• Ensure timely and accurate documentation and compliance with regulatory standards

• Current Registered Nurse (RN) license in the state of Missouri (or compact license) 
• Preferred: Minimum of 1 year of hospice experience 
• Strong communication and assessment skills
• Empathetic and professional demeanor with patients and families
• Comfortable with field-based work and use of electronic charting systems
