Iowa City Hospice is growing! We plan to add another compassionate and dedicated Hospice Aide to our team, full time, M-F 8 AM - 4:30 PM. Our Aides have a passionate understanding for the hospice philosophy and make a focused difference in the quality of life for patients & family members. This critical role provides essential care and support to terminally ill patients to ensure they receive comfort and dignity in their final days. Your responsibilities will include:
Assist patients with activities of daily living such as bathing, dressing, and grooming
Monitor and report any changes in patient's condition to the healthcare team
Support patients and their families emotionally during difficult times
Ensure patient comfort and dignity is maintained at all times
Collaborate with healthcare professionals to create and implement care plans
Maintain accurate and up-to-date documentation of patient care
Adhere to all healthcare regulations and standards
Excellent communication and interpersonal skills
Ability to work effectively in a team environment

ESSENTIAL JOB FUNCTIONS: (including but not limited to): To be performed satisfactorily with or without

Provide personal care and/or environmental assistance for the terminally ill patient. Examples of personal care include: Bathing, toileting, skin or mouth care, shampooing, and observing medication regimen under the supervision of the nurse. Examples of environmental assistance include changing linens, preparing meals or assisting with personal laundry.
Provide respite care in conjunction with provision of personal cares.
Provide modeling of communication and physical care.
Train patient and caregiver techniques of home and hospice care.
Train and assist the patient in the proper use of assistive devises.
Maintain contact with the nurse to assure that care and services are appropriate.
Maintain documentation of patient care activities.
Ensure effective use of resources.
Serve as an educational source to staff, care partners and community.
Ensure implementation of strategic goals.
Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
Participate in agency’s success by ensuring access to our exceptional care. Support and promote agency’s mission.
Participate in the organization’s quality assessment and performance improvement program

Requirements
Requirements:
High School Diploma or equivalent
Completion of the 75-hour home health aide training program, completion of the written and skills portion of the National Homemaker-Home Health Aide Certification Program and an active Iowa Nurse Aide Certificate Number.
Two years of work related experience preferred.
Demonstrates professionalism and decision making abilities, effective interpersonal skills and adhering to ethics, standards, and the support of agency policies and procedures.
Ability to communicate effectively in the English language both verbally and in writing
US work authorization (Required)
Passionate about hospice care with a genuine desire to help others
Reliable, professional and able to start soon after required background checks are completed
Proof of negative TB test is required.
First Aid training
Mandatory Reporter Training
Iowa Driver License, Patient homes are located throughout agency territory (seven counties) with driving requirements that include in-town and highway driving during good and inclement weather.
