Job Description - Hospice Aide (Communities Team)

Description

Are you passionate about providing compassionate care to individuals in need? Do you have a special calling to support patients and families during their end-of-life journey? Hospice of the Piedmont is seeking a dedicated and skilled Hospice Aide to provide personal care services to patients. 

This role involves providing personal care and/or homemaker services to patients under the guidance of a Registered Nurse. The goal is to support both the patients and their caregivers in a way that aligns with the Hospice philosophy, ensuring care is meaningful and compassionate.

In this role you will provide a wide range of responsibilities to ensure the well-being and comfort of patients. Key duties include:

  • Personal Care: Assisting with hygiene, toileting, mobility, medication, and meal preparation.
  • Health Monitoring: Measuring and recording vital signs, fluid intake/output, and reporting changes to the nurse supervisor.
  • Documentation: Maintaining electronic medical records (EMR) and documenting services provided.
  • Housekeeping: Performing light housekeeping duties in the patient area.
  • Patient Environment: Ensuring a clean, safe, and pleasant environment.
  • Infection Control: Following infection control principles.
  • Emergency Procedures: Recognizing and responding to emergencies.
  • Professional Development: Maintaining competency and registration.

Qualifications: 

  • Registered by the Virginia Department of Health Professions as a Certified Nurse’s Aide.
  • One year relevant experience required.
  • Hospice experience preferred.
  • Must demonstrate basic computer skills. 
  • Driver’s license, personal automobile and current automobile liability insurance.

Typical Physical Demands/Working Conditions: 

  • Regular use of hands, reaching, stooping, kneeling, crouching, crawling, talking, and hearing. Frequent standing, sitting, climbing, and balancing. Occasional walking, tasting, and smelling.
  • Ability to lift/move up to 25 pounds and push up to 200 pounds in a wheelchair.
  • Effective telephone communication, keyboarding, and regular use of a computer screen.
  • Frequent travel
