Hospice and Home Health Registered Nurse (RN) - Urgent Hiring

Sunbelt is currently seeking a Hospice and Home Health Registered Nurse for a contract position with a local client in Ontario, OH. Responsibilities:

• Conduct admissions, discharges, resumption of care, and transfers using OASIS
• Manage a caseload of 30 units per week within a 40-hour work week
• Coordinate care with other disciplines, Physicians, Caregivers, and Insurance Companies

Experience:

• Minimum one year of Home Health and Hospice experience within the last year required
• Proficient knowledge and field experience of OASIS required
• Start of Care/Admissions experience required
• Experience in case management, handling at least 5-7 patients per day and 25-30 per week

Education:

• Associate's Degree of Nursing or Bachelors Degree of Nursing
• Ohio Registered Nurse License or compact license
• American Heart Association Basic Life Support certification

Work Hours: 8:00 AM - 5:00 PM, 40 hours per week, with some on-call and weekend requirements
