Hospice & Palliative Care APP

McLeod Health is seeking an experienced Nurse Practitioner to join McLeod Palliative Care Services. This community-based role provides compassionate care to patients with serious chronic and life-limiting illnesses through symptom management, advance care planning, and care coordination. 

We are seeking a provider who resides in or is willing to relocate to the Horry County area, where a significant portion of our patient population receives care.

Position Highlights:

• 100% outpatient, community-based practice with home-based patient visits
  • Monday through Friday schedule, 8:30 AM to 5:00 PM
• No call responsibilities
• Average of 4-6 patient visits per day
• Structured onboarding program with ongoing mentorship and clinical support
• Collaborative interdisciplinary team environment

Team Composition: 

The provider will join an established team consisting of 3 physicians, 8 nurse practitioners, 5 RN care managers, 1 social worker, and dedicated administrative support. Responsibilities include conducting patient assessments, managing complex symptoms, facilitating goals-of-care discussions, evaluating hospice eligibility, and coordinating care across the healthcare continuum. The provider will primarily serve patients in Horry County while supporting regional patient care needs as appropriate. 

Job Qualifications:

• Graduate of an accredited Nurse Practitioner program
• Board certified and/ or eligible for South Carolina licensure
• Minimum of two years of advanced practice experience required
• Strong communication and relationship-building skills
• Experience in hospice, palliative care, geriatrics, home health, oncology, or chronic disease management strongly preferred
