Hospice Case Manager (RN)

As a Hospice RN, you will provide hands-on, compassionate care to patients facing terminal illnesses, offering both clinical expertise and emotional support to patients and families during this critical time.

We are seeking a Full-Time Hospice RN Case Manager to join our dedicated team. This role provides essential care to hospice patients in Boulder City, Las Vegas, and Henderson during weekend hours. The schedule runs continuously from 9:00 AM Monday through 5:00 PM Friday.

Key Responsibilities:

• Provide direct hospice nursing care to patients in their homes.
• Perform comprehensive assessments, symptom management, and pain control.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Offer emotional support and education to patients and their families.
• Maintain accurate clinical documentation.
• Collaborate closely with physicians, hospice aides, social workers, and chaplains.

• Active RN license in the State of Nevada (required).
• Minimum of 2 years of hospice nursing experience (required).
• Current CPR certification.
• Strong clinical assessment and critical thinking skills.
• Compassionate, empathetic, and patient-centered approach.
• Valid driver’s license, reliable vehicle, and auto insurance.
• Ability to travel throughout assigned territory.
