Certified Nursing Assistant (CNA) / Hospice Aide

Pay: $18 – $23 per hour
Job Type: Full-time | Field-based
Schedule: Monday–Friday, 8:30 AM – 5:00 PM

As a hospice CNA, you will provide the hands-on personal care that helps patients feel comfortable, clean, and cared for wherever they call home, whether that's a private residence, assisted living, or a care facility. You'll work closely with a dedicated RN Case Manager as a valued member of our interdisciplinary care team, in a supportive setting with clear expectations and reasonable caseloads. Responsibilities:
- Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care
- Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation
- Assist with dining and nutrition
- Review and follow each patient's plan of care under the direction of the RN Case Manager
- Provide comfort and companionship to patients and support to their families
- Maintain electronic medical records
Qualifications:
- Active Certified Nursing Assistant (CNA) certification (required)
- Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus
- Current CPR certification (preferred)
- Ability to work in an electronic medical record system
- Strong communication skills, including written, verbal, and active listening
- Excellent customer service, problem-solving, and time management skills
- Compassion, patience, and the ability to handle emotional situations with care
- Strong understanding of professional boundaries
- Ability to lift 50 lbs. and good understanding of body mechanics
- Valid driver's license and current auto insurance (required)
