Salary: $60k-65k/yr BOE

Schedule: M-F| 8:00AM-5:00PM +(on call)

Location: Hamilton County, Ohio, and surrounding areas

Requirements of the Hospice Chaplain role:

• 
  

  Ordained, commissioned, or credentialed according to the practices of an organized religious group, or Degree in theology or form of seminary

  
  


• 
  

  Completion of a Clinical Pastoral Education (CPE) program. 

  
  


• 
  

  Comfortable in an interfaith setting

  
  


• 
  

  Valid Drivers License and Auto insurance Coverage

  
  


• 
  

  Reliable transportation 

  
  

About the Hospice Chaplain role:

• 
  

  Conduct a spiritual assessment as part of the comprehensive assessment within 5 days of admission to the hospice program.

  
  


• 
  

  Assist in the development and implementation of the interdisciplinary patient care plan as it pertains to spiritual care.

  
  


• 
  

  Provide spiritual support by ongoing wishes and needs of the patient and family by their religious preferences of definition of spirituality. 

  
  


• 
  

  Work in close collaboration with local clergy when desired by the patient. 

  
  


• 
  

  Maintain and submit written clinical records as deemed by the Care360 Hospice, including the initial evaluation, and the care plan.

  
  


• 
  

  Evaluate the patients' and family's response to, the effectiveness of spiritual counseling. 

  
  


• 
  

  Visit patients in residential locations as needed.

  
  


• 
  

  Maintains adequate and appropriate medical supplies. 

  
  


• 
  

  Participate in IDG and QAPI meetings and activities.

  
  


• 
  

  Conduct religious services of prayer, worship, and rituals for patients/families, as appropriate. 

  
  


• 
  

  Provide funeral or memorial services for patients as requested.
