Hospice Chaplain - Pastoral/Spiritual Counselor

We are currently seeking a dedicated Hospice Chaplain - Pastoral/Spiritual Counselor to enrich the spiritual well-being of our patients and their families. This position involves working closely with a diverse group of patients in the AdvisaCare team located in Metro Detroit.

• Completed graduate theological studies from an accredited seminary or divinity school.
• Experience in hospice care or a related field is highly preferred.
• A demonstrated ability to provide comfort and support to grieving patients and families.
• Strong listening skills and a patient-centered approach.
• Willingness to perform home visits and collaborate with the interdisciplinary team.
• Critical thinking skills to adapt spiritual care practices to individual patient needs.

Key Responsibilities:

• Conduct spiritual assessments to understand the unique needs of each patient and family.
• Develop personalized spiritual care plans in collaboration with local clergy if requested.
• Maintain compliance with all AdvisaCare policies, and applicable regulations.
• Provide spiritual care documentation in Electronic Medical Records accurately.
• Support team members in providing bereavement services as needed.
• Participate in team meetings and contribute to performance improvement initiatives.
• Uphold high standards of customer service and compassion in all interactions.
