Job Overview
We are seeking a dedicated and knowledgeable Hospice Clinical Liaison to serve as a vital link between our hospice services and healthcare providers. The ideal candidate will possess extensive clinical experience, including critical care, emergency medicine, or hospital-based settings, and will be committed to enhancing patient care through effective communication and assessment. This role offers an opportunity to influence patient outcomes positively while working in a dynamic healthcare environment.

Responsibilities

  • Conduct routine facility visits, presentations, and in-services to increase hospice awareness and referrals
  • Identify patients appropriate for hospice services through relationship-building and clinical collaboration
  • Coordinate with the Aspen Hospice clinical and admissions team to facilitate timely evaluations and admissions
  • Build and maintain long-term strategic partnerships with referral sources through consistent communication and follow-up
  • Maintain accurate referral tracking, marketing activity documentation, and follow-up communication
  • Participate in community outreach events, networking opportunities, and healthcare marketing initiatives
  • Represent Aspen Hospice professionally and compassionately within the community
  • Collaborate with Aspen Home Health, Aspen Palliative Care, and Aspen Medical Home teams to support continuum-of-care initiatives
  • Maintain knowledge of hospice regulations, eligibility guidelines, and industry trends
  • Additional Essential Duties & Responsibilities
  • Complete hospice evaluations and informational visits in:

Hospitals (including General Inpatient/GIP evaluations)

Skilled Nursing Facilities (SNFs)

Assisted Living Facilities (ALFs)

Patient homes

Other community care settings as appropriate

  • Collaborate closely with the Aspen Hospice clinical and admissions team to facilitate timely hospice evaluations, eligibility discussions, and admissions.
  • Conduct bedside hospice consultations with patients and families to provide education regarding hospice philosophy, levels of care, goals of care, symptom management, and end-of-life support services.
  • Assist with coordination of hospice admissions upon request by the Aspen Hospice clinical team, including obtaining required documentation, physician certifications, consents, and coordination with referral sources and facility staff.
  • Effectively communicate clinical observations and hospice eligibility findings to the Aspen Hospice interdisciplinary team to support smooth transitions of care.
  • Maintain responsiveness to urgent referral and GIP evaluation requests to support hospital and facility partner relationships and patient care needs.

Skills

  • Proven background in hospital medicine, emergency medicine, or post-acute care environments with a focus on patient assessment and discharge planning.
  • Strong knowledge of medical terminology, physiology, diagnostic evaluation techniques, and inpatient/outpatient workflows.
  • Familiarity with managed care processes, utilization management protocols, and case management strategies.
  • Excellent communication skills for engaging with diverse healthcare teams while maintaining professionalism and clarity.
  • Ability to navigate complex medical records systems while ensuring confidentiality under HIPAA guidelines
