Compensation Spotlight: This role offers a competitive pay range of $90,000 to $140,000 annually, including commission – because stars deserve to shine and be rewarded.

Who's Our Ideal Hospice Clinical Liaison?

1. An LPN or RN with a heart for hospice care and an eye for detail.


2. A relationship-builder, ready to connect with hospital case managers and ensure seamless patient transitions.


3. Someone with a solid understanding of the hospice philosophy and the ability to educate patients and families about their options.


4. A proactive communicator, liaising between the hospital and our Inspire branch, ensuring every referral is handled with care and efficiency.

Qualifications:

1. Current GA LPN or RN licensure without restrictions.


2. Reliable transportation and a valid state driver's license.


3. Experience in a clinical setting, preferably in hospice or a hospital.


4. Strong interpersonal and communication skills.

Your Role as a Hospice Clinical Liaison:

1. Build and nurture relationships with hospital case managers.


2. Respond promptly to hospital referrals, ensuring patients and families receive timely information about hospice care.


3. Educate and guide patients and families about the benefits and processes of hospice placement.


4. Collaborate closely with the Inspire CRR, ensuring a smooth transition for every patient.


5. Act as the bridge, ensuring clear communication between the hospital and the Inspire branch.
