We are seeking a compassionate Home Health Aide (HHA) to provide care and support to hospice patients throughout Tompkins and Cortland Counties.

Responsibilities
- Follow the patient's plan of care as directed by the nurse.
- Report changes in patient condition to the appropriate nurse.
- Document care provided accurately and timely.
- Assist patients with prescribed exercises.
- Provide personal care, bathing, and hygiene assistance.
- Prepare meals and assist with feeding when needed.
- Perform light housekeeping to maintain a safe environment.
- Make beds and help maintain patient comfort.
- Provide respite support for family caregivers.
- Attend required in-services and team meetings.
- Maintain a safe environment and report concerns promptly.
- Perform other duties as assigned.
Requirements
- Current New York Home Health Aide (HHA) Certification required.
- Valid driver's license.
- Reliable transportation.
- Ability to travel throughout Tompkins and Cortland Counties.
