Hospice Intake coordinator

Description of the role:

University Home Care in Livonia, MI is seeking a Hospice Intake Coordinator to join our team. The Hospice Intake Coordinator is responsible for efficiently managing the intake process for patients in need of hospice care, ensuring timely and compassionate care delivery.

Responsibilities:

• Coordinate the intake process for hospice patients, including assessing needs and coordinating referrals.


• Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.


• Document intake process thoroughly and accurately in compliance with regulations.


• Collaborate with interdisciplinary teams to ensure appropriate care plans are developed and implemented.

Requirements:

• Minimum of 2 years of experience in healthcare coordination.


• Minimum of 2 years of experience 


• Strong communication and organizational skills.


• Knowledge of hospice care regulations and processes.


• Ability to work effectively in a fast-paced environment.
