Position Overview:

The Hospice LPN will work closely with the interdisciplinary team to provide compassionate care to patients in their end-of-life journey. This position will be responsible for administering medications, monitoring patient symptoms, and providing emotional support to patients and their families.

Key Responsibilities:

- Administer medications and treatments as prescribed by the physician

- Monitor patient symptoms and report any changes or concerns to the interdisciplinary team

- Provide emotional support and comfort to patients and their families

- Assist with personal care, including bathing, dressing, and grooming

- Document patient care and maintain accurate records

- Collaborate with other healthcare professionals to develop and implement individualized care plans

- Educate patients and families on end-of-life care and support resources

- Maintain a safe and clean environment for patients and staff

- Adhere to all company policies, procedures, and regulations

Qualifications:

- Current LPN license in the state of Illinois

- Minimum of 1 year of experience in hospice or palliative care

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and empathetic nature

- Strong organizational and time-management skills
