Position: Hospice LPN

We are currently seeking a Hospice LPN to join our team in Fairview Heights, IL.

Position Overview:

The Hospice LPN will be responsible for providing skilled nursing care to patients in their homes or in a hospice facility. The LPN will work closely with the interdisciplinary team to develop and implement individualized care plans, provide patient education, and ensure that the physical, emotional, and spiritual needs of patients are met.

Key Responsibilities:

- Perform skilled nursing tasks, including wound care, medication administration, and monitoring patient vital signs

- Collaborate with the interdisciplinary team to develop and implement individualized care plans

- Provide patient education on disease management, symptom control, and end-of-life care

- Administer prescribed medications and treatments in accordance with physician orders

- Monitor and document patient progress, changes in condition, and response to treatments

- Communicate effectively with patients, families, and other healthcare team members

- Provide emotional support and comfort to patients and their families

- Participate in on-call rotation and respond to patient needs after hours as needed

- Adhere to all hospice policies and procedures, as well as state and federal regulations

- Maintain accurate and timely documentation of all patient care activities

Qualifications:

- Current LPN license in the state of Illinois

- Minimum of 1 year of experience in hospice or home health care

- Strong clinical skills and knowledge of hospice care principles and practices

- Excellent communication and interpersonal skills

- Ability to work independently and as part of a team

- Compassionate and empathetic attitude towards patients and their families

- Must be able to pass a background check and drug screening
