Job Description - Hospice LPN

 
qualifications
required
Associate degree or higher

Hospice (2+ years)

1+ years of experience in ANY of the following:

Electronic Medical Record (EMR)

Other EMR

TruChart EHR

Valid Licensed Practical Nurse (LPN) in the state of Illinois (IL)

Do you have a valid Driver's License, Reliable Mode of Transportation, and Auto Insurance (all 3)? (yes)

Have you spoke with a Relode Recruiter about this role (via phone)? (yes)

Are you okay with the required travel for this position? (yes)

Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months? (no)

preferred
Please provide your LPN License Number (Required)

 


Key Responsibilities
● Collaborate to provide management and delivery of patient care plans
● Support patients in Shiloh/Fairview Heights and surrounding areas
● Meet critical deadlines for charting
● Self-manage time and schedule
● Drive to/from patient locations

Schedule and Shift Details
● Work schedule and shift expectations will be provided during onboarding.

Travel
● Occasional local travel within the specified territory
