Partners In Care is seeking an experienced Hospice Program Manager to oversee our inpatient and outpatient hospice services. In this role, you'll ensure high-quality, patient-centered care while leading interdisciplinary teams, maintaining regulatory compliance, and driving operational excellence.

Essential Functions
Leadership & Team Integration
- Lead and support interdisciplinary hospice teams across inpatient and home settings.
- Foster a culture of collaboration, communication, and shared purpose.
- Mentor and develop staff, ensuring alignment with organizational goals.
- Oversight of hospice teams. Regularly provides coaching, counseling, and reviewing job responsibilities. Supports workflow efficiencies and process improvement.

Clinical Oversight
- Ensure high standards of care and regulatory compliance.
- Collaborate with medical and clinical teams to promote continuity and quality of care.
- Monitor clinical outcomes and lead quality improvement initiatives.
- Compassion, empathy, and dedication to patient care.
Operational Management
- Oversee staffing, scheduling, and resource allocation.
- Participate in budget planning and financial oversight.
- Ensure efficient operations while maintaining patient-centered care.
- Maintain compliance with CMS, state, and accreditation standards.
- Lead audits, surveys, and performance improvement efforts.
- Monitor key performance indicators and patient satisfaction.
Communication & Collaboration
- Serve as liaison between staff, leadership, and external partners.
- Facilitate interdisciplinary team meetings and cross-setting communication.
- Represent hospice services in organizational planning and community outreach.
- Travel between Partners In Care sites and business office and community partners as required.
Strategic & Program Development
- Participate in annual budget preparation and co-manage budget adjustments.
- Support program development activities and participate in committees, work groups, and special projects.
- Participate in public relations, community activities, and quality improvement initiatives.
- Clinical Support & Staffing Coverage
- Call duty requirements.
- Perform work to assist with admissions and visits during short staffing periods.

Position Qualifications
- Demonstrates an ability to work collaboratively with other healthcare clinicians, develop a plan of care, knowledge of third-party reimbursement, and negotiate payment rates.
- Self-directed and able to work with minimal supervision.
- Demonstrates excellent observation verbal and written communication skills.
- Continuous learner, maintains clinical skills staying current in nursing skills, equipment, and trends in nursing.
Education and Certification
- Graduate of an accredited School of Nursing program and BSN required. BSN or MSN preferred
- Licensed Registered Nurse in the state of Oregon and license must be in good standing
- Current AHA Basic Life Support for Healthcare Provider certification
- Current automobile insurance required
- As position requires travel between Partners In Care sites and community partners, valid Oregon driver’s license and reliable transportation required
Experience:
- Minimum of five years of in or outpatient clinical nursing experience and two years of supervisory experience required. Inpatient or outpatient hospice or palliative care experience preferred.
- Excellent interpersonal communication skills and the ability to interact with a diverse population; superior nursing process skills; strong analytical, problem solving and decision-making skills; ability to make independent nursing judgments and prioritize workload; knowledge of community services; ability to multi-task and work independently.
- EMR experience required, myUnity preferred. Proficient with Microsoft Office Suite.
- Bilingual in Spanish and English a plus.
Working Conditions:
- May be exposed to infectious and contagious diseases. Contact with patients under wide variety of circumstances.
- May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations.
- Handles emergency or crisis situations.
- Physical demands also include the ability to lift patients, assist with ambulation, walking, carrying, and pushing.
- OSHA exposure category 1: Position includes tasks that involve exposure to blood, body fluids, tissues.
- Required Personal Protective Equipment as conditions warrant: Gloves, gown, mask, mask with shield, goggles, shoe covers, bouffant cap.
