Position Summary:
We are seeking a dedicated and empathetic Hospice Medical Social Worker (MSW) to join our team. This role plays a vital part in supporting patients and families facing terminal illness. The MSW will assess psychosocial needs, provide emotional support, assist with end-of-life planning, and coordinate community resources.

Key Responsibilities:

Conduct psychosocial assessments for hospice patients and families.

Develop and implement individualized care plans in coordination with the interdisciplinary team.

Provide counseling and emotional support related to grief, loss, and coping.

Assist families with advance directives, funeral planning, and access to financial, legal, or community services.

Participate in team meetings and contribute to holistic care planning.

Maintain accurate and timely documentation in accordance with regulatory requirements.

Qualifications:

Master’s Degree in Social Work (MSW) from an accredited institution.

Current [State] licensure (LMSW or LCSW, as applicable).

Minimum of 1 year experience in hospice, palliative care, medical social work, or related field preferred.

Strong interpersonal, communication, and organizational skills.

Ability to work independently and as part of an interdisciplinary team.

Valid driver’s license and reliable transportation.
