Job Description - Hospice Nurse Practitioner

We are seeking a compassionate PRN Hospice Nurse Practitioner to join our team in Harrisonburg, VA. The primary responsibility of this role is to conduct required face-to-face visits for patients entering their third and subsequent benefit periods to assess their continued eligibility for hospice care. In addition, the Nurse Practitioner may also perform clinical assessments and start-of-care visits as needed, providing support during the admission process. This role plays a vital part in ensuring continuity of care, clinical oversight, and compassionate support for patients and their families throughout the hospice journey.

A Brief Overview of Responsibilities :  



  • Conduct required face-to-face visits for patients in their third and subsequent benefit periods to assess ongoing hospice eligibility

  • Perform assessments and Start of Care visits as needed

  • Educate patients and families on the hospice plan of care and provide supportive guidance

  • Collaborate with physicians and the interdisciplinary team to review clinical findings and ensure care continuity

  • Complete, accurate, and timely clinical documentation

  • Perform additional duties as needed to support patient care and agency operations



Qualifications: 



  • Must be a Nurse Practitioner with a current license to practice in the Commonwealth of Virginia

  • At least one year of hospice experience is preferred but not required.
