Job Description - Hospice Nurse Practitioner-PRN-GLEN CARBON, IL

Hospice Nurse

Practitioner-PRN

at our New Start-Up in Glen Carbon, IL location.

As a Hospice Nurse Practitioner,

you will impact the lives of our patients by -

Complementing attending physician care
Act as a medical resource person to the IDG
Performing Face-to-face visits as needed
Evaluating hospice eligibility as assigned in consultation with the hospice IDT
Assuring the patient receives appropriate measures to control uncomfortable symptoms
Patient visits to wherever the patient calls home
Collaborating with IDG to ensure the needs of the patient are met
Supports clinical and administrative staff in providing compassionate care
Ensures that the patient meets the medical criteria for hospice admission based upon available diagnostic and prognostic indicators, related diagnosis(es), current medical findings, current medication and treatment orders, information about medical management of the patient's conditions unrelated to the terminal illness
Attending ADG conferences as needed
Participated in plan of care development and managing oversight of medications and treatment
Documenting care provided in the patient's clinical record providing evidence of progression of the end-stage-disease process
Maintaining current knowledge of the latest research and trends in hospice care and pain/symptom management
Working in a team approach
Participating in performance improvement programs, as needed
Reviewing and developing protocols for treatment and proposing the most current options for interventions
Demonstrating knowledge in communications and counseling patient/family in dealing with end-of-life issues
To become a Hospice Nurse Practitioner,

you will need -

Must have completed an accredited Nurse Practitioner Program
Current advanced nursing licensure in MO
Has experience with end-of-life care. Preferred a minimum of 2 years of clinical experience in Oncology, Pain & Symptom Management, Palliative Care, Hospice, healthcare in a hospital or physician practice setting
Maintains controlled substances registration with sate and federal authorities
Must have the ability to demonstrate self-confidence and positive attitude toward self and others
Not excluded from the Medicare program
The ability to communicate well, both verbally and in writing
Must be a licensed driver with an insured automobile in good working order, and clear driving record
