Overview:

Our clients are in search of compassionate and seasoned Hospice Registered Nurses to become a member of their dedicated care team. The responsibilities include managing and delivering personalized patient care plans, providing assistance to patients throughout Eastern Pennsylvania, and ensuring the maintenance of timely and accurate documentation. The ideal candidate will possess an active RN license in Pennsylvania, have prior hospice experience, and exhibit strong organizational skills along with the ability to manage their schedule independently.

Qualifications:

• Active Registered Nurse (RN) license in Pennsylvania.
• Minimum 2 years of RN experience, hospice experience preferred.
• Experience with EMR systems; familiarity with TruChart EHR is a plus.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong organizational, communication, and patient-care skills.

• Maintain accurate and timely documentation in electronic medical records (EMR).
• Provide compassionate support to patients and their families in various PA locations.
• Deliver and manage patient care plans in accordance with best practices.
• Effectively manage time and schedule to meet patient needs and deadlines.
