Hospice Registered Nurse Axxess Experience

University Home Hospice Inc is seeking a compassionate and experienced Hospice Registered Nurse with Axxess experience to join our team in Livonia, MI. This role involves providing exceptional hospice care to patients across Wayne, Oakland, and Macomb counties, ensuring quality and comfort during end-of-life care.

Responsibilities

• Deliver skilled nursing care to hospice patients in their homes throughout Wayne, Oakland, and Macomb counties.


• Utilize the Axxess platform to document patient care accurately and efficiently.


• Collaborate with interdisciplinary teams to develop and implement individualized care plans.


• Provide education and support to patients and their families regarding disease processes, symptom management, and end-of-life care.


• Assess patient conditions regularly and adjust care plans as needed.


• Maintain compliance with state and federal regulations related to hospice care.

Requirements

• Valid Registered Nurse (RN) license in the state of Michigan.


• Experience with hospice care and proficiency using the Axxess electronic health record system.


• Strong interpersonal skills with the ability to work effectively with patients, families, and multidisciplinary teams.


• Ability to travel within Wayne, Oakland, and Macomb counties.


• Compassionate and patient-centered approach to end-of-life care.
