About the Role:
We are seeking a passionate and dedicated Hospice Registered Nurse case manager to join our team at One Health Home Health in San Ramon and Livermore, CA. This role offers the opportunity to make a meaningful impact on the lives of our patients and their families, while working in a supportive and collaborative environment.
Responsibilities:
- Provide skilled nursing care to hospice patients in accordance with the established plan of care
- Assess and monitor patient's condition and collaborate with the interdisciplinary team to develop and implement individualized care plans
- Administer medications and treatments as prescribed by the physician
- Provide education and support to patients and their families
- Document all patient care activities in a timely and accurate manner
Requirements:
- Current RN license in the state of California
- Minimum of 2 years of nursing experience, preferably in hospice or home health
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Compassionate and empathetic demeanor
Duties:
- - Assess patients' medical conditions and develop individualized care plans
- - Collaborate with healthcare professionals to coordinate and manage patient care
- - Monitor patients' progress and make necessary adjustments to care plans
- - Provide education and support to patients and their families regarding their medical conditions and treatment options
- - Conduct regular evaluations to ensure the quality and effectiveness of patient care
- - Utilize critical thinking skills to make informed decisions regarding patient care
- - Adhere to HIPAA regulations and maintain patient confidentiality at all times
- - Collaborate with insurance providers to ensure appropriate coverage for medical services
- - Assist with discharge planning and coordination of post-hospital care
