Hospice Registered Nurse (RN) Case Manager

Advanced Home Health and Hospice of Arizona provides services within patient's homes valley-wide. We are seeking a compassionate and experienced Hospice RN Case Manager to join our dedicated team. In this role, you will provide comprehensive end-of-life care, coordinate patient-centered plans, and support families during a critical time. Responsibilities include managing a caseload of hospice patients, performing clinical assessments, collaborating with interdisciplinary teams, and ensuring compliance with care standards. The ideal candidate is a licensed RN with strong clinical skills, hospice or palliative care experience, and a deep commitment to patient dignity and comfort.

Typical Physical Demands:

• Full range of body motion, including handling and lifting of patients
• Manual dexterity, finger dexterity, and eye-hand coordination required
• Ability to occasionally lift and carry items weighing 50 pounds or more
• Corrected vision and hearing within normal range
• Possible stressful working conditions or irregular hours due to the nature of the job and the health stage the patient is currently in
• Exposure to communicable diseases
• Must have a current driver’s license and ability to drive to and access patients' homes.

• Valid RN License in the state of Arizona
• 1 year experience as an RN -Hospice experience is not required

Working Conditions:

• The majority of work is performed in providing patient care services in the patient’s home.
• Exposure to communicable diseases is common.

Job Type: Full-time (Monday-Friday 8:30 am-5 pm with occasional on-call)
