Hospice RN

University Home Care Inc is seeking a compassionate and dedicated Hospice Registered Nurse to join our team, providing exceptional end-of-life care to patients and their families. This role involves delivering high-quality nursing care, symptom management, and emotional support across Wayne, Macomb, and Oakland counties. The ideal candidate will have a strong background in hospice care, excellent communication skills, and a commitment to enhancing the quality of life for patients during their final stages.

Responsibilities

• Provide comprehensive hospice nursing care to patients in their homes across Wayne, Macomb, and Oakland counties.


• Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.


• Manage symptoms and pain effectively to improve patient comfort and quality of life.


• Educate patients and families on disease progression, medication management, and available resources.


• Document patient care and progress accurately in compliance with regulatory and company standards.


• Coordinate with physicians, social workers, chaplains, and other healthcare professionals to ensure holistic patient care.


• Respond promptly to patient and family concerns, providing emotional support and guidance.

Requirements

• Registered Nurse (RN) license in the state of Michigan.


• Minimum of 2 years’ experience in hospice or palliative care nursing preferred.


• Strong knowledge of end-of-life care principles and symptom management.


• Ability to travel within Wayne, Macomb, and Oakland counties.


• Excellent communication, interpersonal, and organizational skills.


• Compassionate approach with the ability to support patients and families emotionally.


• Current CPR certification.
